For a number of large tertiary care hospitals, receiving referral or transfer patients from less specialized hospitals is a critical component of their healthcare mission.  The ‘transfer center’ within the hospital, however, faces many challenges.   

Nurses, physicians and staff involved in the transfer center must manage multiple phone calls, collect accurate clinical and demographic patient information, coordinate referring and admitting physicians, locate appropriate bed or surgical/treatment suites, and often arrange for transportation such as helicopters and ambulances.   Today, most of this is done manually making it difficult to efficiently gather, manage, measure and facilitate the communication of critical information, and patients, between institutions.
