TeleTracking’s Operational Platform Extends Across the Care Continuum with the Launch of Two New Community Solutions

PITTSBURGH (March 5, 2018) – TeleTracking Technologies, Inc., the KLAS Patient Flow Category Leader for the seventh consecutive year, is further extending its operational platform across the care continuum with the launch of two new community solutions.  Community Placement, which improves post-acute access―giving case managers and discharge planner’s real-time visibility to available post-acute beds, and giving patients a more seamless transition to the next step in their care. And Community Scheduling and Workflow, a cloud-based solution that provides patient self-scheduling and workflow tools for ambulatory settings.

It also allows for better overall care coordination, potentially aiding readmission rates.

Finally, from the post-acute providers’ perspective, Community Placement provides a direct referral pipeline for a system’s post-acute facilities, increasing volume and decreasing the time spent competing for referrals.
