TeleTracking’s Community Access™ Portal Expands Patient Access Capabilities by Bringing Visibility to Community Care Teams
PITTSBURGH (October 27, 2016) – TeleTracking Technologies, Inc., the KLAS patient flow category leader, is helping health systems strengthen relationships with community providers through the launch of Community Access™ Portal.
Having already implemented more than 100 system-wide transfer centers across the country since 2009, Community Access Portal builds on TeleTracking’s proven patient access solutions by improving communications between health system access centers and referring care teams. Participating community care teams are now able to initiate the admission process online with minimal patient information, reduce the number of phone calls during the process, and track milestones after the patient is admitted.
